Mr. Jack Smith, age 70 of Hartsville, TN died Friday, February 1, 2013 at Vanderbilt University Medical Center, Nashville, TN. Preceded in death by, Grandson, Logan Blake Smith. Survived by: Wife, Mrs. Thelma Smith-Hartsville, TN; 2 Sons, Mr. Roger (Tracy) Smith-Lebanon, TN & Mr. Joey (Tonya) Smith-Lafayette, TN; Daughter, Mrs. Lois (Charlie) Woodard-Lafayette, TN; 5 Grandchildren, Dalton Cole Smith, Joseph & Matthew Woodard, Tyler & Brayden Smith; Great Grandchild, Kenzi Woodard 2 Brothers, Mr. Alex (Ruth) Smith-Hendersonville, TN & Mr. Jim Smith-Hartsville, TN and Sister, Mrs. Sue (Olie) Allan-Gallatin, TN. Funeral services will be held in the chapel of Anthony Funeral Home at 2 PM Tuesday, February 5, 2013 conducted by Bro. J.C. Cox. Interment will be in the Hartsville Memorial Gardens. Visitation with the family will be from 1 to 8 PM Monday and 10 AM until service time Tuesday. In lieu of flowers, donations may be made to the American Cancer Society.
